Title: Social Work Professors Seek to Improve Access to Community Services

Dr. Jason Dauenhauer and Dr. Kristin Heffernan, Professors of Social Work, are partnering with Lifespan of Greater Rochester and the Monroe County Department of Human Services Adult Protective Division (APS) to improve client access to community services, improve health outcomes, and prevent the recurrence of abuse. This two-year project is funded by the Administration for Community Living with a budget of $733,060. As part of a subcontract to SUNY Brockport, Heffernan and Dauenhauer are responsible for assisting with implementation, data collection, and overall program evaluation.

Specifically, this project will pilot a model of co-locating an aging service care manager with APS staff for on-site consultation and joint assessment of clients to facilitate access to the full array of Older Americans Act-funded and other community services, and to offer intensive care management by project care managers at Lifespan once the crisis that prompted a referral to APS is stabilized.
