Re-Evaluation Requests for Registered SAS Students
Students who are already registered with SAS, who believe they are encountering disability-related barriers, may submit a “re-evaluation request” in Accommodate to start an interactive process. Re-evaluation inquiries are opportunities to request new/different accommodation(s). The Re-Evaluation Request Form submission will trigger SAS to review documentation currently on file and/or evaluate newly submitted documentation (if applicable, and if uploaded to Accommodate). If the appropriate supporting documents are present, SAS will engage the student in the interactive meeting process to determine accommodation adjustments. Students will be contacted by SAS if the documentation on file does not have a direct connection to the current request so that the student is fully informed when deciding upon their next steps.

- Attach new documentation, if applicable
- Click “Submit”
