To receive accommodations, an individual must engage in the self-identification process with Student Accessibility Services (SAS) and provide appropriate documentation.
Step 1: Submit a Registration Form

Step 2: Provide Eligibility Documentation
Educational Documentation Pathway:
If you received accommodations in high school, you may provide a copy of your most recent Individualized Education Plan (IEP), Psychological evaluation, or a 504 plan. Due to the interactive nature of the intake process, we recommend that you provide as much recent documentation as possible to help substantiate requests for accommodations.
Declassification documentation, coupled with the disability classification, can also be used as grounds for eligibility.
Medical Documentation Pathway:
If you have a medical diagnosis that rises to the level of a disability, we recommend that you have your physician, or licensed counselor, complete our Clinical Assessment Form for a comprehensive evaluation and thorough documentation of your needs. Students submitting a request for an assistance animal as a housing-related accommodation should have an appropriate diagnostician complete our Assistance Animal Application.
Another option is to have an *appropriate diagnostician provide a disability diagnosis with a description of the condition and its severity, measures to arrive at the diagnosis, prognosis, expected duration, how this condition affects you (functional limitations), recommendations for accommodations, provide direct connections between the functional limitations and the recommendations for accommodations, and include the side effects of prescribed medication if applicable.
*An appropriate licensed diagnostician or qualified clinician, can be a primary physician, nurse practitioner, physician’s assistant, licensed mental health professional, etc. The diagnostician must (1) have an established patient relationship with the student, (2) have provided treatment for the condition, and (3) be an impartial individual who is not a family member of the student.
Community Rehabilitation Documentation Pathway:
You may also submit functional reports from community-based rehabilitation providers such as ACCES-VR and Commission for the Blind for registration purposes.
*All documentation is assessed for completeness and appropriateness under Section 504 of the Rehabilitation Act of 1973 and the Americans with Disabilities Act of 1990, as amended. Such assessment is necessary to establish a student’s eligibility for academic accommodations and/or auxiliary aids and services.
Step 3: Schedule an Appointment
After your documentation is reviewed, we will contact you, via email, to schedule an appointment for an interactive intake meeting. At this meeting, we will talk about your strengths and needs as a learner and develop a plan of support in the form of an accommodation letter.
Step 4: Obtain Your Official Accommodation Letter
You’ll leave this meeting empowered to disclose your accommodation letter, to self-advocate for your needs, and to take advantage of the community and college-wide resources that are available to help you each your goals.
When Can I Expect to Hear Back?
Please Note: It may take up to four weeks to review and/or process your information/documentation. Freshman and new students are strongly encouraged to request accommodations from SAS as early as possible prior to attending SUNY Brockport. Current students may request accommodations at any time but are encouraged to make their requests for accommodations as soon as they become aware of a need for one due to the four-week timeline.
